To help us make your first visit as smooth and efficient as possible, please bring the following information with you:
What to Bring

Current Medication List: If you are currently taking any medications, please bring an up-to-date list, including the names and dosages of your medications.

Insurance Information: If you have dental insurance, please bring your insurance information so we can assist with verifying your coverage and benefits.

Previous Dental Records & X-Rays: If you are transferring from another dental office and would like us to request your previous X-rays or dental records, please let us know. We will be happy to assist with obtaining them.
What to Expect at Your First Visit
Your initial appointment will include a comprehensive consultation with one of our dentists. During this visit, the dentist will discuss your dental health, explain any diagnoses, and review recommended treatment options with you.
When appropriate, we may also schedule a cleaning with one of our skilled dental hygienists as part of your initial visit.
To allow enough time to complete your new patient forms and gather the necessary information for your dental chart, please arrive at least 15 minutes before your scheduled appointment time.
